The church is Gothic Revival and built of brick. Adolphus Druiding of Chicago designed the reconstruction that followed the tornado of 1896, so what stands now is largely of 1897, and six ancillary buildings were listed with it. The parish school opened in 1869 across Eleventh Street.

Bohemians founded it in 1854. St. John Nepomuk was the first national Bohemian parish established in the United States, a Czech newspaper was printed in one of its buildings, and the city named it a landmark in 1971 before the National Register listed it the following year. The parish closed in 2005.
